As part of the IHCA agenda, Michele Self, MA-SLP, Customer Success Manager at Real Time, will present the session, Successful Discharge Begins on Admission – Improve Outcomes through Effective Care Navigation and Live Analytics. The session will take place Tuesday, September 15, 2026, from 1:30–2:30pm CT. Attendees will learn how establishing discharge goals at admission, leveraging real-time clinical insights, and coordinating interventions across disciplines can strengthen interdisciplinary collaboration, improve patient outcomes, and support successful transitions of care.

Participants will learn how to recognize clinical changes that may warrant reassessment, optimize the use of Interim Payment Assessments (IPAs), and help accurately capture patient acuity for appropriate PDPM reimbursement.
